Pre-Register

Currently Trebol Families:
Pre-register your family and players.

For Players New to Trebol:

You can come to a tryout session

without pre-registration or pre-register

here to help us communicate with you.

Before you start, please note that

the following information is REQUIRED

during Pre-Registration:

Emergency Contact Information (Name, Phone)
Child's Physician (Name, Phone)
Insurance (Company Name, Phone, Policy Holder Name and Policy Number)

REMEMBER:

In Step 1 - User id is for the family and

Primary Contact is a Parent

In Step 2 - Add multiple players to your

account by clicking the + sign.

Finally - YOU MUST COMPLETE ALL 7 STEPS.

Once you Click the Register button you

may print the registration for your

records.

Peak to Peak K-12 School

Check the "Tryout Schedule"

June 1 - June 5

Review

Team's Invitation,

Respond

and Select Fees

Within 24 hours of the last tryout,

each player, U11 and above,

will receive an e-mail

including his or her placement.

Each player will need to

accept or decline that placement

within 24 hours of the initial

notification by clicking "Accept Offer".

Fees will be selected at this time and are based on the Team assignment of

each player in your family. Please

note the fee code included in

your team assignment e-mail message.

drop off a birth certificate

and pay your fees by check.
